Gardena Student Athletes,

Welcome to the 2024-2025 school year of sports.

Please complete all of the required athletic paperwork below.

Athletic Clearance Confirmation

1) Create an account or sign on to www.athleticclearance.com

2) Complete all forms, print confirmation, and sign

Medical Insurance Card

1) Copy of Medical Insurance or purchase insurance at kandkinsurance.com.

LAUSD Pre-Participation Physical Evaluation

1) Download and print the form (English / Spanish).

2) Parent/Guardian will complete Page 1. Any "Yes" answer must be explained.

3) Doctor will complete Page 2 including signature and stamp.

Visit the Clinic at Gardena High School or call 424-306-8280 to make an appointment.

CIF Transfer Form (only for a sophomore, junior, or senior student athlete who was not at Gardena High School last school year)

All student athletes must maintain at least a 2.0 GPA and adhere to the Student Code of Conduct and Athlete Code of Conduct.
